How it works.
The supported self-assessment will identify the areas of your life you find difficult.
- The assessment will take about one and a half hours to complete; you have up to 30 days to complete it.
- You will need to register using an email address. This will enable you to save your form and return to it at any time.
- When you see a question with a red asterisk (*), this means it is a mandatory question and must be answered to allow you to proceed with the form.
The assessment can be completed by someone with care and support needs or it can be completed by someone else on their behalf where appropriate (e.g. relative, advocate, or other professional).
The self - assessment
The assessment is split into two parts.
Part one
The wellbeing questionnaire. This is a chance to consider your situation through a set of targeted questions. Upon completion, a range of advice and information will be provided for you to consider and explore.
Part two
If you wish to proceed, asks a number of in-depth questions relating to your daily life.
The supported self-assessment will cover:
- Your living situation.
- basic financial information
- your physical, mental and emotional wellbeing
- how your needs affect your work, leisure, education, wider family and relationships
- what you would like to achieve in your day-to-day life
- whether you need any support
- whether you qualify for any help from the council
