You can only use Home First if you have been referred by a health care worker (for example, hospital staff or a care worker) and are returning home from hospital.
Home First provides reablement support on behalf of Suffolk County Council.
Home First will work with you to create a support plan to help you live at home.
The plan will include goals that have been agreed with you. Help can last up to six weeks or end sooner if you reach your goals. Most people meet their goals between two and four weeks.
You'll get help with daily tasks (for example cooking and bathing) and with doing more things yourself.
